SUD Care Navigator
Navigation. Coordination. Support. Improving Access and Reducing Gaps in Care for Priority Individuals
Mid-State Health Network is committed to supporting priority populations through increased awareness of services, timely access to care, and coordinated support provided by the SUD Care Navigator. Through navigation and coordination, individuals are connected to the most supportive treatment and community resources based on their unique needs.
Who We Support
- Individuals referred by the Michigan Department of Corrections (MDOC)
- Individuals at risk of losing custody of a child or involved with the child welfare system
- Pregnant individuals in need of treatment
- Individuals who use substances intravenously
How Support Works
Support may begin through formal referrals or direct outreach from the individual or community partner. From first contact through connection to care, support includes:
- Identifying needs through a personalized screening
- Coordinating referrals and supporting timely access to care
- Communicating across providers and systems (with consent)
- Helping reduce barriers and supporting service connections
- Assisting with transitions in care and ongoing coordination
Supporting Those Who Support Priority Populations
The SUD Care Navigator works alongside community partners to support timely, coordinated, person-centered care, including collaboration with:
- MDOC: Supports treatment access through the MDOC referral process
- MDHHS / Child Welfare: Access and coordination to treatment
- SUD Providers: Timely admissions and referral coordination
- Families & Supports: Guidance and resource connection
- Community Partners: Collaboration across justice and community systems
