How to Request a SUD Screening-Referral Process
- To obtain MDOC priority status an individual must be referred using the following documents:
- Ways to submit a referral:
- Email: Send the MDHHS 5515 release and CFJ-306 forms via encrypted email to: MDOCreferrals@midstatehealthnetwork.org
- MDOC Portal: Coming soon...a new, secure way to send substance abuse treatment referrals. Enroll in the MDOC Referral Portal for access to:
- A secure system for sending protected health information
- Access forms and submit instantly
3) Referral Response-Next Steps:
- SUD Screening Options:
- Person completes a direct call to SUD Care Navigator, 517-299-0089 or Mid-State Health Network Access, 844-405-3095
- SUD Care Navigator calls the person directly, or if currently incarcerated, coordinates a date/time with the referring agent to complete the screening from jail
- Referring Agent will receive an email with the details of the screening outcome; if the person meets medical necessity for residential treatment, a referral and warm transfer will occur to the person’s preferred treatment center
- The Person, Agent and Treatment Center will coordinate treatment placement; confirmation of admission to treatment and treatment progress will be emailed to the agent from the treatment center, if the individual completes the MDHHS 5515 to consent to communication
