How to Request a SUD Screening-Referral Process

  1. To obtain MDOC priority status an individual must be referred using the following documents:

         MDDHS 5515 Behavioral Health Release Form

         CFJ-306 Substance Abuse Treatment Referral form  

  1. Ways to submit a referral:​​​​​​​
  • Email: Send the MDHHS 5515 release and CFJ-306 forms via encrypted email to: MDOCreferrals@midstatehealthnetwork.org
  • MDOC Portal: Coming soon...a new, secure way to send substance abuse treatment referrals. Enroll in the MDOC Referral Portal for access to:
    • A secure system for sending protected health information
    • Access forms and submit instantly

3)      Referral Response-Next Steps:

  • SUD Screening Options:
    • Person completes a direct call to SUD Care Navigator, 517-299-0089 or Mid-State Health Network Access, 844-405-3095
    • SUD Care Navigator calls the person directly, or if currently incarcerated, coordinates a date/time with the referring agent to complete the screening from jail
  • Referring Agent will receive an email with the details of the screening outcome; if the person meets medical necessity for residential treatment, a referral and warm transfer will occur to the person’s preferred treatment center
  • The Person, Agent and Treatment Center will coordinate treatment placement; confirmation of admission to treatment and treatment progress will be emailed to the agent from the treatment center, if the individual completes the MDHHS 5515 to consent to communication
